Management of Challenging Patients Through the Continuum of Care to Ensure Timely, Cost Effective, Quality Outcomes
This workshop will provide the participant with insights and tools regarding the design and development of an effective continuum plan for chronic and challenging patient populations. Discussions and learning will include how to ensure the appropriate skills and competencies of your case management team, review and development of optimal and effective workflows across the sites, and how to enable technology to meet both communications and process improvement needs.
OBJECTIVES:
- Participant will be able to identify opportunities for challenging patients across the continuum
- Participant will learn about the core and contemporary competencies needed by staff and leadership to drive change and align interdisciplinary teams
- Participant will be able to recognize workflows that are not effective and build new plans for connectivity between sites and practitioners
- Participants will learn how to leverage technology to improve communications, predict patients’ needs into the future and identify processes to need improvement

About the Speaker:
Bonnie Geld, MSW, President of The Center for Case Management, brings over 20 years’ experience in the area of Case Management with a focus on design of longitudinal case management programs, strategic design and development of transition programs, process improvement, team building, and successful integration of social work and RN roles.
She has been the System Director for Case Management Services for a five-hospital system in Western Massachusetts, Director of Care Management in Minneapolis and Houston, and has developed and implemented successful Care Management programs in other states, both in Large Adult Systems in Oklahoma and Children’s Health Systems in Texas. She has launched a variety of educational programs and workshops on promoting lean actions, team interventions, rapid cycle assessments, and integrated care planning.
